How do I cite a home instruction exercise sheet?

Hmm ... citing "odd" sources can be tricky, and giving advice can be tricky, too, if you cannot see the source, but I will do my best.
I assume you are going to be citing this exercise sheet in APA?
I think we should treat this as an unpublished, or informally published work. I think it is safe to assume that no matter how hard I look in my traditional library catalogs or databases I would not be able to find it? So, just like an interview, it is not "recoverable" by the reader of your paper.
These are the basics of what you will need.
Author - is there a person or company list on the sheet who takes credit for the information?
Date of publication - if not, then use (n.d.). for "no date."
Title of the exercise sheet.
Description of information - APA lets you sometimes give extra info. This is presented in brackets after the title [Home exercise instruction sheet].
The last piece of the citation, if available, would be the publisher information -place & publisher if available, just publisher, if not. It might end up that your author is the same as the publisher [if you decide the author is a company].
So, a final citation might look like -
White, T. (2011). My, those stomach crunches hurt! [Home exercise instruction sheet]. Acme Exercise.
