I am going to assume that you are wishing to cite this an APA; however, sometimes AMA is used for PT, so please email back if you wish to know how to cite this in AMA.

I found on page 72 of the document a reference (#3), which cites a similar document as this one only in AMA. Basically it lists the APTA as the author, has the date and the title of the document and then gives the URL of the PDF thus treating this similarly to a web site. I think that would work, too. It is entirely acceptable to have an organization rather than a person as the author as author simply is stating who is responsible for the content, and for this it would be the APTA.

This isn't a journal article, so that's why there isn't a journal name, volume, DOI, etc., and that's okay, too. There is though the APTA even saying it was prepared by them stating responsibility and the date right away on the first page.

So I think this would be the best (I can't do tabbing on this, so please apply the spacing rules in your Word doc). In case it doesn't translate, the title of the PDF should be in Italics with the capitalization rules applied for APA (only first word of title and after colon except for proper nouns and adjectives).
