If we think of this a web page as opposed to an article, we can simplify the citation and still provide all the information needed for the reader to find the original source.
1 - Author: A citation is almost always going to start out with the author statement. Here you have a corporate, or institutional author, as opposed to an individual. Informally, it looks like the School of Nursing at The College of New Jersey. But, we need to cite them as they name themselves.
