How do i cite an online book with no author or date?

I don't know how to put an in text and reference page citation to the book under this link: http://health.prenhall.com/nieswiadomy/pdf/NIESWIADOMY1.pdf

I am 90% certain this chapter came from this book. It might be possible it came from the sixth edition, but it seems to match the fifth.

Nieswiadomy, R. M. (2008). Foundations of nursing research (5th ed.). Upper Saddle River, NJ: 

            Pearson/Prentice Hall.

But, that doesn’t necessarily solve our problem. I would normally advise not to cite a source when you don’t know the author, title, or date. And if you do cite it as an online source, the URL will not lead the reader to the above information, so that is also wrong.

This is the best advice I have, given with some reservation.

a). Cite the work as a print source, using the information listed above.

b). Then (and this is important) request the book through InterLibrary Loan - https://mplus.mnpals.net/vufind/Record/006621731

c). Once you have your hands on the book, it validates your citation. If you don’t take this step, while I believe that the information is correct, we enter a grey area of the ethical world – we are starting to make up a citation and that is what worries me.

There is always the option, too, of “walking away” and just not using this source.
