I don't know if we would find an "official" answer to this anywhere in the APA vault.

I would stay with Raleigh. Creating a book would have the intellectual part, the design and layout, and the physical printing. Once upon a time that would have all been done in one house. In modern publishing the intellectual, design, & layout could be done by anyone, anywhere with a computer. You can be a publisher without owning a press and loving the smell of ink in the morning.

My guess is, then, lulu.com creating the intellectual content and then jobbed off the physical printing to the place in Kentucky, which is why it is "made" in Lexington as opposed to the traditional term "printed." Your paper is more concerned with the intellectual content, so I think Raleigh is the right choice.
